New Study Finds Xylitol Nasal Spray Dramatically Reduced Bacterial Burden and Recurrent Ear Infections in Children
Results strengthen the case for nasal hygiene as a simple way to clean and support one of the body’s most important gateways

New research suggests we should be paying more attention to another major gateway into the body: the nose and upper airway.A newly published multicenter clinical study found that children using a nasal spray containing xylitol and grapefruit seed extract experienced an 87.5% reduction in bacterial isolates in the upper airway, compared with only a 22% reduction among children using a placebo.
The difference is striking because both groups were washing their noses. Washing alone appeared to have an effect, but the reduction in bacterial isolates was nearly four times greater with the xylitol-based formulation.
The study adds to a growing body of research suggesting that nasal hygiene, or washing the nasal passages and supporting the body’s natural clearance mechanisms offers an important additional approach to respiratory health.
Dramatic Differences in Bacteria, Ear Infections and Antibiotic Use
Published in Medical Research Archives, the study included children ages 1–15 with a history of recurrent acute otitis media, commonly known as middle-ear infections.
Researchers at 10 outpatient centers in the Czech Republic compared children using a xylitol and grapefruit seed extract nasal spray for three months with children using a placebo. A total of 154 children completed the treatment period.
The results went well beyond bacterial counts.
Only 38.8% of children using the xylitol-based spray experienced another acute ear infection, compared with 90.5% in the control group.
Antibiotic use for ear infections was also substantially lower:
11.3% in the xylitol group versus 35.1% in the control group.
Overall antibiotic use for any infection was 32.5% versus 52.7%.
These findings are important because bacteria that contribute to childhood ear infections colonize the nose and nasopharynx before reaching the middle ear.
Reducing that bacterial burden at its source provides a new way of thinking about prevention.
Why Xylitol?
The findings are consistent with earlier laboratory research showing that xylitol can affect important respiratory bacteria and viruses. Multiple studies have shown that xylitol blocks adhesion, or the ability of bacteria and viruses to adhere to tissue in the airway. Bacteria like Strep Pnuemo, H. Flu, viruses like sars-cov-2, RSV, and many others.
That makes xylitol particularly interesting for nasal hygiene.
A nasal spray can provide two complementary actions:
First, wash the nose. Fluid helps physically remove mucus, allergens, pollutants, bacteria and other unwanted material.
Second, make it more difficult for certain microorganisms to establish themselves. Research suggests xylitol can interfere with the growth and adhesion of some respiratory bacterial and viral pathogens.
The principle is simple:
Wash away what you can. Make it harder for what remains to stay.
That isn’t sterilization. It’s hygiene.
Nasal Hygiene Should Be Viewed Like Oral Hygiene
For generations, we have understood the importance of oral hygiene.
The mouth has saliva and its own natural defenses, yet we still brush our teeth every day because physical cleaning supports those defenses.
The nose also has sophisticated natural defenses—mucus, cilia, immune mechanisms and a complex microbiome.
But we rarely think about cleaning it.
Oral hygiene helps manage the microbial environment of the mouth. Nasal hygiene can help manage the microbial environment of the upper airway.
The objective is to regularly remove unwanted material and support the body’s natural defenses, without sterilizing or destroying the commensal microbiome.
The new study provides particularly interesting evidence for that concept. An 87.5% reduction in pathogenic bacterial isolates compared with 22% in the control group suggests that what we use to wash the nose may matter as much as the act of washing itself.
These results add to decades of research on xylitol’s interaction with respiratory pathogens and provide another reason to investigate nasal hygiene as a simple, cost effective approach to promoting a healthy upper airway.
For more than a century, public health has taught us two simple habits:
Wash your hands.
Brush your teeth.
As we learn more about the importance of the upper airway, it is time to add a third:
Wash your nose.
